  1. Best all-in-one plan

    CareStack

How we ranked

Six rules, in priority order.

A product earns a pick because a specific sourced fact beats the same fact for others in its category. Review ratings only break ties. No vendor pays for placement, a ranking position or a mention.

  1. Currently sold Legacy, sunset and acquired products are left off every list.
  2. Published pricing A vendor-published price outranks quote-only in the same category.
  3. Contract and exit terms Term length, notice period, and the cost of getting data out.
  4. Deployment fit Server, cloud or native macOS decides awards like "keep a server".
  5. Specialty focus Built for the specialty, rather than listing it alongside nine others.
  6. Review ratings Capterra, G2 and Software Advice, only to break a tie.

Where are Dentrix and Eaglesoft?

Both are among the most widely installed systems in US dentistry. Neither publishes a price, and a ranking built on what vendors put in writing cannot place a product whose cost arrives only after a sales call. They sit on their own profiles instead, with everything they do publish.

  • 35,000+ practices Dentrix Henry Schein One · the vendor's own claim, checked Sep 26, 2026
  • Nearly 30,000 users Eaglesoft Patterson Dental · the vendor's own claim, checked Sep 26, 2026

How we ranked these

We start from the same fact sheet every software profile on this site uses: published pricing, contract length and notice period, what happens to a practice’s data if it leaves, deployment (cloud, server or hybrid), and which specialties a vendor says it supports. A product earns a spot on this list because a specific field in that sheet beats the field for other currently sold products in the same category, not because of a general impression of the interface. The full method is on our methodology page.

Every starting price in the box above is a product’s own published figure, not a third-party guess, unless it’s marked as an estimate. “Starting price” means the cheapest regular plan a vendor lists, never an intro rate, an add-on, or a fee charged only when a practice leaves. When a vendor doesn’t publish a number at all, the box says so instead of guessing at one.

Cloud, server or hybrid

Several of the picks above run entirely in a browser: CareStack, Oryx, Denticon and TDO among them. There’s nothing to install locally and no server to patch, so staff can work from a second location or from home. The trade-off is that the practice’s data lives on the vendor’s servers, which makes the export terms in each pick’s contract box matter more than they would for a server-based system.

Three picks run on a server the practice owns: Open Dental, Dentrix Enterprise and OMSVision. The practice keeps the database, and Open Dental’s is an open MySQL file it can query directly. Owning the server also means owning the hardware, its backups, and any Windows or Windows Server upgrades the vendor eventually requires to keep running the current version.

MacPractice DDS sits in between: it keeps its database on the practice’s own Macs, with MP.Go giving browser access on top. ABELDent runs the same product on a local server (Local Plus) or in Microsoft Azure (Cloud), at the same list price either way.

What published pricing buys

Most products in our database are quote-only, which means a practice negotiates without knowing what anyone else pays. CareStack, Open Dental, Denticon, Oryx and ABELDent all put a number in writing before a sales call, and each shows what that number includes. CareStack’s Essentials tier, for instance, includes native imaging, eligibility checks, claims and texting at $829 a month, with no separate step to turn any of it on.

Published pricing doesn’t mean fixed pricing forever. Denticon’s terms let Planet DDS adjust the price at each renewal, with written notice. Treat a starting price as the floor for a negotiation, not the final bill once a practice adds locations, providers or specific modules, and check a pick’s own contract terms for how its renewal pricing works before you sign.

Specialty coverage

General dentistry is the default case for most systems in our database, which is why CareStack, Denticon and Open Dental all cover it alongside several other specialties. Orthodontics, oral surgery and endodontics get thinner coverage from general-purpose systems, which is why Ortho2 Edge Cloud, OMSVision and TDO earn separate picks. Each is built for its specialty rather than listing it alongside general dentistry. A specialty practice should start with the dedicated system before comparing headline prices against a generalist; the workflow gap usually costs more in the first year than the subscription does.

Groups, DSOs and multi-location practices

Dentrix Enterprise and Denticon both target multi-location and DSO practices, but from different directions. Dentrix Enterprise runs every location on one central SQL Server database that the group hosts itself, which suits an organization that already runs Windows infrastructure and wants one database rather than a cloud login. Denticon is the cloud alternative: no server to manage, and reviewers cite running several locations from one system. Planet DDS lists API integrations with more than 50 third-party products and built-in analytics.

Contracts and getting data back out

Contract terms vary pick by pick, so read each one rather than assuming they match. Denticon renews automatically every year, with 30 days’ notice to skip that renewal or 60 days’ notice to cancel for convenience. Oryx and Open Dental both start with a 12-month commitment and then move to month-to-month, so neither locks a practice into a second annual cycle. CareStack’s arrangement is set on each Order Form, and either side can end it at any time on 30 days’ notice. ABELDent’s Microsoft Marketplace subscription runs one year and is set to auto-renew, though we couldn’t confirm that renewal beyond the marketplace listing itself.

Dentrix Enterprise’s service plans are sold as yearly or monthly contracts, quoted by phone, and MacPractice DDS, Ortho2 Edge Cloud, OMSVision and TDO don’t publish anything on this at all. Wherever a plan does auto-renew, put the notice date on a calendar well before it arrives; missing it commits a practice to another cycle at whatever price applies by then.

Data export terms vary more than pricing does. Some vendors let a practice pull its own data at any time from inside the software; others require a written request within a set window after leaving, and a few charge a separate fee for imaging exports specifically. Read the export clause on a pick’s software profile before deciding to switch, not after.

Where are Dentrix and Eaglesoft?

Dentrix and Eaglesoft are both active products with large user bases by their vendors’ own counts: Henry Schein One claims more than 35,000 practices on Dentrix, and Patterson claims nearly 30,000 Eaglesoft users. Neither made this list because of how our criteria work out. Henry Schein One and Patterson publish no prices for either one, so in each award category a product with a published price, or one built for that specific niche, ranked ahead. Both still suit plenty of practices, especially ones already running them on a working Windows server with IT support behind it.

We cover both in full: see the Dentrix profile, the Eaglesoft profile, Dentrix vs Dentrix Ascend, and the other head-to-heads on our comparisons page.

Easy Dental is missing for a different reason. It is sunset in our data, meaning Henry Schein One no longer sells or supports it, and inactive products are excluded from every list.

Review-site ratings from Capterra, G2 and Software Advice factor into these picks only as a secondary signal, never the deciding one, and we never blend a third-party rating into a vendor’s own published numbers. Where a rating appears on a pick’s software profile, it carries a “third-party source” label and a link back to the review site itself.

Using this list

Start with the award that matches your situation rather than the position in the ranking; a solo Mac practice gets more use out of the Mac pick than from whichever product sits at position one. If two picks fit equally well, the Find my PMS quiz scores every active product in our database against your specialty, practice size, budget and stated priorities, and the five-year cost calculator turns a pick’s published pricing into a real total once you enter your provider and location counts. Every price on this page is checked on the same schedule as the rest of the site: a source link, a checked date, and a re-check once that date passes 90 days.

Frequently asked questions

What's the best dental practice management software overall?

It depends on what you need. If you want a published price with no sales call first, start with CareStack, Open Dental or ABELDent. If you're opening a new practice, Oryx's $0/month startup offer removes cash-flow risk in year one. If you're a group or DSO, compare Dentrix Enterprise's on-premise, single-database model against Denticon's cloud model. The picks above cover the rest of the common cases: keeping a server, running Macs, or a specialty like orthodontics, oral surgery or endodontics.

Is there a free dental practice management system?

ABELDent Freemium is $0/month, limited by the free Microsoft database it runs on. Phone support, one-on-one training, updates and data migration are billed at ABELSoft's regular rates, and patient texting and online forms aren't included, so it suits a very small or very new office more than one that's already growing.

Which dental PMS lets a new practice start for close to nothing?

Oryx's Pro and Automate plans run $0/month for new practices, for a $1 setup fee, until the practice reaches 200 patients or 12 months, whichever comes first. Beyond our picks above, tab32's Alpine Start-Up plan is $125/month in year one for up to 3 providers, with no setup fee, rising to $225/month from year two.

Can I keep running my own server instead of moving to the cloud?

Yes. Open Dental and Dentrix Enterprise both run on a server you own. Open Dental's database is open MySQL you can query directly; Dentrix Enterprise runs every location on one central SQL Server database, which Henry Schein One built for multi-location and DSO practices specifically.

Does any vendor pay to be ranked on this page?

No. Every pick above is scored against the criteria on our methodology page: published pricing, contract and exit terms, deployment and platform fit, specialty coverage, and review-site ratings as a secondary signal. We re-check every number on a schedule and update this page when the underlying data changes.
