Best ortho-only system with a published price
Ortho2 Edge Cloud
Ortho2 Edge Cloud is built for orthodontics and nothing else, and it is the only ortho-only product on this list that publishes a subscription figure: $384/month as a starting price. A new-graduate offer runs $100/month. The catch is onboarding, listed at $6,995, with a $5,495 sale price also published, so the first-year cost is dominated by a one-time fee rather than the subscription. Ortho2 publishes no contract term, so ask for it in writing before you sign.

Lowest published price
tab32
tab32 publishes $225/month for its Alpine Established Practice plan, and $125/month for the first year on Alpine Start-Up, which is the cheapest confirmed orthodontic-capable figure we found. Its pricing page says no servers and no contracts, and lists no setup fee on the Start-Up package. Claims are billed at $0.20 each and AutoRemind reminders and two-way texting run $69/month after year one, so the headline is genuinely a headline rather than a bundle.

Best offer for a new practice
Oryx
Oryx puts a startup practice at $0/month on the Pro or Automate plan, and $400/month on the AI plan instead of $1,399. After the offer, Pro is $650/month with each additional provider at $100. Texting, eligibility checks, claims submission, online forms and online scheduling all come with the base product rather than as add-ons. The initial contract is 12 months, then month-to-month, which is a shorter commitment than most of this list.

Best all-in-one, and the highest rated
CareStack
CareStack publishes both of its tiers: Essentials starts at $829/month and Intelligence at $1,299/month. Patient texting, insurance eligibility checks, claims submission and online scheduling are part of the base product, so a per-seat add-on list does not appear later. It holds 4.8 out of 5 from 219 reviews on Capterra, the strongest rating of any product on this page. Its standard terms let either party terminate on 30 days' notice, with or without cause.

Best established ortho and pediatric practice
Cloud 9
Cloud 9 serves orthodontics and pediatric dentistry and is one of the few systems on this list built for both. Planet DDS publishes no price for it; Capterra lists a starting figure of $500/month, which is a third-party estimate rather than a quote. The number to ask about is leaving: XVWeb patient and imaging data export is published at $995, and non-renewal needs 30 days' notice while cancelling for convenience needs 60 and makes the rest of the term payable.

Cheapest confirmed monthly price
MOGO Cloud
MOGO Cloud publishes $250/month, the lowest confirmed base price of any orthodontic-capable system we track other than tab32's established-practice rate. Patient texting, insurance eligibility checks, claims submission and online forms come with the base product. It holds 4.4 out of 5 from 59 reviews on Capterra. MOGO publishes no setup fee and nothing about what it costs to leave, so both belong on the list of questions for the call.

How we ranked
Six rules, in priority order.
A product earns a pick because a specific sourced fact beats the same fact for others in its category. Review ratings only break ties. No vendor pays for placement, a ranking position or a mention.
- Currently sold Legacy, sunset and acquired products are left off every list.
- Published pricing A vendor-published price outranks quote-only in the same category.
- Contract and exit terms Term length, notice period, and the cost of getting data out.
- Deployment fit Server, cloud or native macOS decides awards like "keep a server".
- Specialty focus Built for the specialty, rather than listing it alongside nine others.
- Review ratings Capterra, G2 and Software Advice, only to break a tie.
Quick verdict
If you run an orthodontic practice and you want to see a price before a sales call, the list is short. Ortho2 Edge Cloud is the only orthodontics-only system we track that publishes a subscription figure. tab32 and MOGO Cloud are cheaper, but they are general systems that support orthodontics rather than products built around it.
The rest of the ortho-specific market, Dolphin Management, topsOrtho, OrthoTrac and Sensei Cloud, publishes nothing. That is not a judgement about the software. It means you cannot compare them on price without asking each one for a quote, and you cannot check a quote against anything.
What is different about orthodontic software
An orthodontic practice runs on treatment plans that last years, contracts paid in instalments, and imaging that sits at the centre of the clinical record rather than beside it. The billing is the part most general systems handle badly: a general product bills a visit, an orthodontic product bills a contract.
That is why the specialist products exist, and why so many of them are old. Dolphin Management and OrthoTrac both run on a server in the practice. OrthoTrac is carried here as legacy: Carestream’s master terms say a support plan cannot be ended without cause in the first 12 months, and ending early means paying the rest of that period.
The newer cloud products come at it from the other direction. CareStack, Oryx, iDentalSoft and MOGO Cloud are general systems that list orthodontics among their specialties. Whether that is enough depends entirely on your treatment-card workflow, which is a question to put to a demo rather than to a comparison table.
Published pricing, and what it leaves out
Nine of the fourteen orthodontic-capable systems we track publish at least one figure. Six of those nine are stated by the vendor; three appear only as third-party estimates on listing sites, which do not say where their numbers came from.
The published figures range from $225/month for tab32’s Alpine Established Practice plan to $1,399/month for Oryx AI. Ortho2 Edge Cloud sits in between at $384/month, but its onboarding is listed at $6,995, with a $5,495 sale price published alongside it. On a twelve-month view that onboarding fee is larger than the subscription, which is the kind of thing a monthly headline hides.
Three things are missing from almost every published price on this list: a setup fee, a contract term, and anything about what leaving costs. Cloud 9 is the exception on the last one, and only because the figure is unflattering: $995 to export patient and imaging data out of XVWeb.
How to use this list
The ranking is a starting shortlist. A product that publishes its price has told you something useful about how it sells, which is why published pricing weighs heavily here, but it says nothing about whether the software fits how you run bracket inventory or sibling accounts.
Take two or three from this page into the cost calculator, add the onboarding and conversion figures from each product’s profile, and compare five years rather than a monthly rate. Then ask every vendor the same four questions: what is the initial term, how many days’ notice to cancel, what does a full data export cost, and does it include images.
Frequently asked questions
What is the best practice management software for an orthodontic practice?
It depends on whether you want a system built only for orthodontics or a general system that supports it. Ortho2 Edge Cloud is ortho-only and publishes $384/month. tab32 is a general cloud system at $225/month that lists orthodontics among its specialties. CareStack is the highest rated at 4.8 out of 5 from 219 reviews and publishes both of its tiers. Dolphin Management and topsOrtho are also ortho-focused, but neither publishes a price.
How much does orthodontic practice management software cost?
Of the 14 orthodontic-capable systems we track, 9 publish at least one price. Confirmed figures run from $225/month for tab32's Alpine Established Practice plan to $1,399/month for Oryx AI. Ortho2 Edge Cloud publishes $384/month plus onboarding listed at $6,995. The products that publish nothing include Dolphin Management, topsOrtho, OrthoTrac and Sensei Cloud, so a quote is the only way to price those.
Do I need ortho-specific software, or will a general system do?
Both exist on this list. Ortho2 Edge Cloud and topsOrtho are built for orthodontics alone. CareStack, Oryx, tab32, iDentalSoft and MOGO Cloud are general systems that list orthodontics among the specialties they support. The practical difference is usually treatment-card and imaging workflow rather than billing, so the question worth asking a vendor is what an ortho visit looks like in their software, not whether the box is ticked.
Which orthodontic systems publish their prices?
Nine of the fourteen we track publish at least one figure, and six publish a price the vendor states itself rather than a listing site: CareStack, iDentalSoft, MOGO Cloud, Ortho2 Edge Cloud, Oryx and tab32. Cloud 9, Dentrix Ascend and XLDent appear only as third-party estimates. Dolphin Management, Epic Wisdom, OrthoTrac, Sensei Cloud and topsOrtho publish nothing at all.
