Best startup offer
Oryx
Oryx's startup offer puts a new practice at $0/month on the Pro or Automate plan, for a $1 setup fee, until it reaches 200 patients or 12 months, whichever comes first. The startup rate on the AI plan is $400/month instead of the usual $1,399. After the offer ends, Pro runs $650/month for up to 2 providers, and the initial contract is 12 months, then month-to-month.

Cheapest start-up plan with no patient cap
tab32
tab32's Alpine Start-Up package is $125/month for the whole first year, covering up to 3 providers, with no setup fee and no cap on patient count, unlike a startup rate that ends once a practice crosses a patient threshold. Its own pricing page lists no contract term for the package ("No servers. No contracts."). The rate rises to $225/month from year two. Claims are billed separately at $0.20 each, and reminders and two-way texting are free in year one, then $69/month.

Best all-in-one flat price
Archy
Archy charges one flat rate per location: $899/month regularly and $400/month for new practices until they reach 400 patients or one year. That price includes unlimited users, providers, claim submissions, eligibility checks, texting, native imaging, online forms and online scheduling, so a new practice isn't shopping for those separately.

Best published pricing to grow into
CareStack
CareStack's Essentials tier starts at $829/month with native imaging, eligibility checks, claims and texting already included, and licenses are set by location and provider rather than by staff seat. Its practice-size coverage runs from startups through DSOs, so a new practice can stay on the same platform as it adds providers or opens a second location.

Best if you want to own the server from day one
Open Dental
Open Dental's support costs $199/month for the first 12 months, then $149/month, for up to 3 dentists, with each extra dentist at $20/month. The database is open MySQL that the practice controls on its own hardware. The initial term is 12 months, then month-to-month, with a 90-day money-back guarantee on support fees.

Best flat per-practice price
iDentalSoft
iDentalSoft's Starter plan is $395/month per practice, and the vendor says the price doesn't change with the number of providers or users. It includes texting, eligibility checks, unlimited e-claims, online forms and native imaging. Cancelling before the initial term ends costs the lesser of the remaining contract payments or three months of fees.

How we ranked
Six rules, in priority order.
A product earns a pick because a specific sourced fact beats the same fact for others in its category. Review ratings only break ties. No vendor pays for placement, a ranking position or a mention.
- Currently sold Legacy, sunset and acquired products are left off every list.
- Published pricing A vendor-published price outranks quote-only in the same category.
- Contract and exit terms Term length, notice period, and the cost of getting data out.
- Deployment fit Server, cloud or native macOS decides awards like "keep a server".
- Specialty focus Built for the specialty, rather than listing it alongside nine others.
- Review ratings Capterra, G2 and Software Advice, only to break a tie.
Why a new practice needs a different list
A practice that hasn’t opened yet has no legacy data to convert and no existing staff habits to protect, so the picks above lean on cost in year one, what’s bundled into a single price, and how soon a system moves off a promotional rate. That’s a different set of priorities from an established office weighing a switch. CareStack, which also makes our general best dental practice management software list, is here for a different reason. It has no new-practice rate, so the $829/month Essentials starting price a startup sees is the same published figure an established office sees. The final price still depends on locations, chairs and providers.
Every fact below comes from the same sourced data as the rest of the site: a vendor’s own pricing page or terms of service, with a link and the date we checked it, or a labeled third-party estimate where the vendor doesn’t publish a number.
Startup rates and when they end
Three of the six picks above have an explicit new-practice rate that’s lower than their regular price. Oryx drops to $0/month on Pro or Automate until a practice reaches 200 patients or 12 months, whichever comes first, for a $1 setup fee. tab32’s Alpine Start-Up plan runs $125/month in year one for up to 3 providers before rising to $225/month. Archy charges $400/month per location for a new practice until it reaches 400 patients or one year, against a regular rate of $899/month.
Read the fine print on each offer before you plan a budget around it. Oryx’s own pricing page ties the cutoff to whichever limit comes first, patient count or time, and a fast-growing new practice can hit 200 patients well before the 12-month mark. Archy’s contract terms let the vendor set the full commitment on the order form and generally don’t allow ending the agreement early except for cause, so the startup rate is a discount inside a binding term, not a trial.
What comes bundled versus billed separately
Archy and iDentalSoft both charge one number that already includes texting, insurance eligibility checks, claims submission and online forms, so a practice that’s also hiring staff and buying equipment isn’t managing a separate vendor relationship, invoice and support line for each of those. CareStack takes the same approach at its Essentials tier: eligibility, claims and texting are part of the $829 base price rather than options you discover later in a demo.
tab32 and Open Dental price the core platform separately from usage-based add-ons. tab32 bills claims at $0.20 each on top of the subscription, and its AutoRemind texting tool is free in year one before becoming a $69/month line item. Open Dental prices texting and its eServices bundle as add-ons on top of the support fee. Neither approach is wrong, but it changes what your first invoice totals compared to the advertised starting price, so ask for a sample invoice before you sign.
Cloud versus a server you own
Four of the six picks, Oryx, tab32, Archy and CareStack, run entirely in a browser, so a new practice doesn’t need to buy server hardware before opening. For a startup with a tight opening budget, that removes a capital expense most of the picks that require a Windows server would otherwise add on day one.
Open Dental is the exception on this list: it runs on a server the practice buys and maintains, in exchange for an open database the practice fully controls and one of the lowest published monthly support fees among active, general-purpose systems in our database. That trade only makes sense if the new practice already has, or plans to hire, IT support for that server.
Contracts to watch before opening day
Open Dental and Oryx both settle into a month-to-month arrangement after their first 12-month term, which limits how long a new practice is locked into a rate that might no longer fit as it grows. iDentalSoft’s terms take the opposite approach: leaving before the initial term ends costs the lesser of the remaining contract payments or three months of fees, on top of whatever the practice already paid.
tab32’s own pricing page states plainly that the Start-Up package carries no contract, though its Established and Summit tiers, aimed at practices that have already grown past the startup stage, are negotiated separately. Get the actual term length for any plan in writing before your opening date, since a verbal “no contract” claim on a marketing page isn’t binding on its own.
Imaging and hardware you still have to buy
Software pricing is only part of a new practice’s opening budget, and none of the picks above include the sensors, cameras or CBCT units that generate the images a system displays. Archy and CareStack both build imaging into their platform natively, so there’s no separate imaging software license to add once the hardware is in place. Oryx does the same and lists Pearl and Overjet as imaging-analysis integrations on top of its native viewer. tab32 also includes native imaging in its base price.
We don’t have a sourced imaging fact for Open Dental either way, so a new practice should confirm sensor and CBCT compatibility directly with Open Dental and its equipment vendor before buying hardware. Texting comes through its eServices bundle, which is billed separately from the base support fee. iDentalSoft’s Starter plan includes native imaging, alongside a long list of third-party imaging integrations for practices that already own specific sensor brands.
What we didn’t include
We limited this list to systems whose practice-size data explicitly covers startups, which excluded some products in our broader database that are strong for an established office but don’t publish anything aimed at a practice that hasn’t opened yet. It also excludes every product carrying a status other than active, for the same reason our general best-of list does: a new practice needs a vendor selling and supporting the product today, not one being phased out. If a product you’re considering isn’t on this page, check its own software profile for the same pricing, contract and export facts we used to build this list, or run it through the quiz below.
Using this list
Run each pick’s published starting price, plus what it bundles, against your opening-year budget rather than choosing on brand recognition alone. If your specialty isn’t general dentistry, or your practice size doesn’t match what’s listed here, the Find my PMS quiz scores every active product in our database, not only the six above, against your specialty, size, budget and stated priorities. Once you have real quotes in hand, the five-year cost calculator turns each pick’s published pricing into a total you can compare side by side, and our methodology page explains exactly how we chose the picks on this page.
Frequently asked questions
What should a new dental practice look for in practice management software?
Cash flow in year one, and how much comes bundled at that one price. Oryx and tab32 both publish reduced start-up rates that remove the biggest early cost, and Archy and iDentalSoft charge one flat number that already includes texting, eligibility checks and claims rather than billing them as separate add-ons. Compare the published starting price, what's included at that price, and the initial contract term before you sign.
Is there a dental PMS with no setup fee for a new practice?
tab32's Alpine Start-Up package lists no setup fee, and Oryx's startup offer has a $1 setup fee rather than the full onboarding charge it lists elsewhere. Open Dental's support plan has no separate setup fee either, though its 12-month initial term still applies.
Can a new practice start on a month-to-month contract?
Open Dental and Oryx both move to month-to-month after their initial 12-month term. tab32's own pricing page for the Start-Up package says "No servers. No contracts," though Established and Summit plans are arranged during a sales consultation instead.
Do these systems charge extra once a new practice grows past its first year?
Usually, yes. tab32's Start-Up plan roughly doubles, from $125 to $225 a month, once year one ends. Oryx's startup rate ends at 200 patients or 12 months and reverts to the regular plan price. Archy's new-practice rate of $400/month per location reverts to $899/month at 400 patients or one year. Budget for the regular price before you commit to the startup rate.
